Frenulum refers to a connecting tissue between two parts of the body. Frenulum typically restricts the motion of one part it is connected to. There are several frenulum in the body including the small connecting tissue under the penis that connects the head of the penis and the foreskin. When an uncircumcised man is aroused and is having sex, the foreskin on the penis head moves behind the penis head. In men with short frenulum (Frenulum Breve), it becomes difficult for the foreskin to move behind the penis completely. This causes extreme pain and makes having sex difficult.
How do you prepare for Frenuloplasty surgery?
Your doctor/sexologist will advise you to stop smoking before scheduling the surgery. Frenuloplasty is a day care procedure that doesn’t take longer than 30 minutes. Smoking causes other complications and delays healing of the injury (the incision made during the surgery).
Secondly, you will be advised not to eat or drink anything and see the doctor for surgery on empty stomach. The surgery involves administration of local or full body anesthesia. Your stomach needs to be empty before you get anesthesia. It is also advisable that you get someone along with you to the hospital / clinic so that they can take you home safely. You should not drive yourself home after the surgery as the effects of anesthesia will take a long time to wear off.
How is frenuloplasty surgery done?
The procedure is done at doctor’s office / clinic. You will be given local or full body anesthesia as per the discretion of the doctor. The frenulum will be cut off or extended as needed by the doctor. It was done using a scalpel previously but with the advent of laser technology, doctors today use laser to extend or cut the frenulum. This helps in minimizing the blood loss, speeding recovery process and lesser pain. Once the extension or removal of frenulum is done, the area is stitched and bandaged.
An alternative to frenuloplasty is circumcision, a procedure where the foreskin is completely removed. There are other benefits of circumcision procedure including reduction of sensitivity of the penis glans (helping in extending the time of intercourse / delaying ejaculation), prevention of acquiring sexually transmitted infections etc.
Taking care of yourself after frenuloplasty surgery
The stitches made will be either dissolve on their own or they may fall off after a week after healing. During the healing process, you will need to see if the wound is bleeding. (Typically, in laser frenuloplasty, there will be lesser bleeding). If the wound’s bleeding doesn’t stop, rush to your doctor immediately.
Avoid contact with water. If the wound requires dressing, visit your doctor and get the dressing done. Complete the course of antibiotics and use the pain medication as required and advised by your doctor. Avoid intercourse at least for a period of 6 weeks. Once healed, visit your doctor and see if everything has healed as required.
Risks and complications that can be expected after frenuloplasty
Frenuloplasty is a minor surgery. So, the risks and complications will be very minor. Keep the area clean to avoid any infection. In case of abnormal pain or bleeding, you should see your doctor immediately. Other than these in rare cases, we haven’t seen any patient with complications after performing frenuloplasty.
